Auto Accident New Patient Instructions

Print and complete Auto Accident New Patient forms. Contact our office (850-422-2225) to schedule an appointment.

On your first visit, you will need to bring:

  • Completed Auto Accident New Patient forms
  • Auto insurance card
  • Driver’s License
  • Copy of the police report (if applicable)
  • Insurance claim number
  • Personal injury (not property) adjuster name and contact information
  • X-ray report and films or disk (if applicable)

If you have questions or need assistance on how to obtain these items our office staff will be happy to assist you. Please contact our office prior to your appointment day if you need our help.


Group Health Insurance New Patient Instructions

Effective January 1, 2009, we no longer file group health insurance. However, our office will assist you in filing for reimbursement from your insurance company. We accept cash, check or credit card for payment. We accept all major credit cards.

Our goal at Tallahassee Chiropractic Sports Medicine and Rehab Center is to help you achieve the quality of life you deserve. It does not make sense for us to be hindered in this goal by the insurance companies. Recent changes by the insurance industry have made it impossible for us to continue as a ‘provider’.

Our office fees have been reduced to make your upfront expenses similar, if not cheaper than your current policy. We have various types of treatment plans for short and long term care including maintenance.

We look forward to returning to the days when the doctor could just be the doctor and not a businessman. I want simply to be YOUR doctor, without the insurance companies’ interference and restrictions governing my and your decisions.

Medicare New Patient Instructions

Print and complete the Medicare new patient forms. Contact our office (850-422-2225) to schedule an appointment.
On your first visit, you will need to bring:

  • Completed Medicare New Patient forms
  • Medicare card
  • Driver’s License (if applicable)
  • Secondary or supplemental insurance card
  • Diagnostic reports, any X-ray reports and films or disks (if applicable)

Please be aware that Medicare only covers specific services for Chiropractic care. Furthermore, not all secondary or supplemental insurance covers expenses not paid for by Medicare.

Self Pay New Patient Instructions

Print and complete the Self Pay New Patient forms.  Contact our office (850-422-2225) to schedule an appointment.

We offer a 25% discount to patients that do not have health insurance.  After your needs/wants are assessed, there are several payment options available for further discounts.  Or you may choose to simply take advantage of the 25% discount at each follow-up visit.ractic.co
